The Forms tab on Courses is a faculty list of every form you can use in simulation classes: forms you created, plus forms shared with your programs. Open it from Courses in the top navigation, then click Forms (/classes?tab=forms). This is the simulation-class forms list. Field placement evaluations and site paperwork live in FieldX — see Building forms if that’s what you need.

What’s on the list

The heading explains the scope: Forms you own and forms shared with your programs. Each row shows:
  • Form title and description
  • Created By (for example, You)
  • Actions, including View/Edit and a delete control where you have permission
Click the form title or View/Edit to open that form at /forms-and-reports/forms/.... Use Search forms… to filter by title or description. The All forms dropdown filters the list by visibility.

Create a form

Click Create New Form (top right). That opens the form builder at /forms-and-reports/forms/new. If you have no forms yet, the empty state offers Create your first form.
After a form exists, you attach it to a class as a form assignment from inside that class — not from this tab. Open the class from My Courses, then create an assignment.
